Abstract

|
암환아 어머니 43명과 일반환아 어머니 124명을 대상으로 환아 질환에 대한 불확실성의 정도, 스트레스, 불안 정도를 분석한 결과는 다음과 같다.
환아 연령으로 조정된 암환아 어머니의 환아 질환에 대한 불확실성 점수는 평균 56.0이었고, 일반환아 어머니의 불확실성 점수는 평균 50.4였다. 암환아 어머니와 일바환아 어머니의 환아 연령으로 조정된 스트레스 점수는 각각 평균 106.0. 70.5였다. 환아 연령으로 조정된 암환아 어머니의 불안 점수는 평균 53.6, 일반환아 어머니는 평균 49.9점이었다. 불확실성 (p<0.05)과 스트레스 (p<0.01)에서 암환아 어머니의 점수가 일반환아 어머니의 점수보다 유의하게 높았다.
단변량분석 결과, 환아와 어머니의 일반적 특성에 따른 환아 어머니의 환아 질환에 대한 불확실성은 암환아 어머니의 경우 일반적 특성들과 유의한 관련이 있는 변수는 없었고, 일반환아 어머니의 경우에서는 환아의 연령이 7세 이상인 경우, 환아의 주관적 질환 위중도가 높다고 생각할수록 불확실성 점수가 높았다 (p<0.01). 스트레스는 암환아 어머니의 경우 어머니의 주관적 건강상태가 나쁠수록 스트레스를 많이 받았고(p<0.05), 일반환아 어머니의 경우 환아의 연령이 많을수록(p<0.01), 환아의 질환이 위중하다고 생각할수록(p<0.05), 환아의 병원입원 횟수가 많을수록(p<0.01), 어머니가 건강하지 않을수록 스트레스를 많이 받았다(p<0.01). 환아 어머니의 환아 질환에 대한 불안의 정도는 암환아의 경우는 환아의 연령이 많을수록, 가정의 경제적 형편이 나쁠수록 불안 점수가 높았고(p<0.01), 일반환아의 경우는 환아의 연령(p<0.01), 환아의 병원입원 횟수(p<0.05) 등이 유의한 관련성이 있었다.
일반환아 어머니의 불확실성 점수를 종속변수로 한 다중회귀분석에서 환아의 나이가 7세 이상인 경우가 1-6세인 연령대보다, 그리고 환아의 질환이 위중하다고 생각할수록 불확실성 점수가 높았다(p<0.01).
암환아 어머니의 스트레스 점수를 종속변수로 한 다중회귀분석에서 어머니의 주관적 건강상태가 나쁠수록 스트레스 점수가 높았으며(p<0.01), 일반환아 어머니의 스트레스 점수를 종속변수로 한 다중회귀분석에서는 환아의 입원 횟수가 많을수록(p<0.05), 어머니의 주관적 건강상태가 나쁠수록(p<0.05), 불확실성 점수가 높을수록(p<0.01) 스트레스 점수가 높았다.
암환아 어머니의 불안 점수를 종속변수로 한 다중회귀분석에서 환아의 나이가 7세 이상인 군이 1-6세인 군보다(p<0.05), 불확실성 점수가 높을수록(p<0.01) 어머니의 불안 점수가 높았으며, 일반환아 어머니의 불안점수를 종속변수로 한 다중회귀분석에서는 불확실성 점수가 높을수록 어머니의 불안 점수가 높았다.(p<0.01).
따라서 환아 어머니의 스트레스와 불안을 최소화하기 위해서는 환아 질환에 대한 불확실성을 줄이는 동시에 간호를 하는 어머니의 건강상태를 함께 고려하는 간호중재가 필요할 것이다.

This study was performed to examine the uncertainty, stress, and anxiety of the mother with a sick child. A self-administered questionnaire survey was carried out for 167 mothers.
The mean uncertainty score adjusted by age of child was 56.0 in the mothers with the children suffering from cancer and 50.4 in the mothers of sick children with other disease than cancers. The mean stress score adjusted by age of child was 106.0 in the mothers of the children with cancers and 70.5 in the mothers of sick children with other disease than cancers. The mean anxiety score adjusted by age of child was 53.6 in the mothers of the children with cancers and 49.9 in the mothers of sick children with other disease than cancers.
In bivariate analysis, uncertainty score in the mothers of sick children without cancers was significantly different according to the age of the child, mother´s perception of the severity for child´s disease. The stress score was significantly different according to the perceived health status of the mothers of the child with cancer and significantly different according to the age of the child, mother´s perception of the severity for child´s disease, admission numbers of child, and the perceived health status of the mothers of sick children without cancers. Anxiety levels were significantly different according to the children´s ages, economic status of the family of the mothers of children with cancers and significantly different according to the ages and admission numbers of the children in the mothers of sick children with other disease than cancers.
In multiple regression analysis, uncertainty scores in the mothers of sick children without cancers were higher in the cases of older than 7 years of ages and when mother´s perceived severities for children´s diseases were increasing. Stress scores in the mothers of children with cancers were higher when mother´s perceived health status was not healthier and in the mothers of sick children without cancer, stress scores were higher as admission numbers of child and uncertainty scores were increasing, and when mothers´ perceived health status was not healthier. Anxiety scores in the mothers with children suffering from cancers were higher in the cases of older than 7 years of age and as uncertainty scores were increasing. In the mothers of sick children without cancers, anxiety .scores were higher as uncertainty scores were increasing.
In consideration of above findings, it appears that controlling the uncertainty of the mothers with sick children is necessary for an improved and efficient nursing intervention.
